📌 Acute Traumatic Osteochondral Injuries
✅ Acute Traumatic Osteochondral Lesions
- Definition: Encompasses a wide range of traumatic injuries to the cartilage and subchondral bone.
- Spectrum: Includes bone bruises and chondral, subchondral, and osteochondral fractures.
- Description: The degree of depression and displacement of osteochondral fractures should be described.
✅ Bone Bruise or Bone Contusion
- Definition: Trabecular microfractures manifesting as bone marrow edema on MR images.
- Characteristics: Without contour abnormalities or a discrete fracture line. Edema appears speckled and poorly defined.
✅ Subchondral Fracture
- Definition: Fracture not involving articular cartilage or subchondral bone plate.
- Characteristics: Linear areas of low T1 and T2 signal intensity, frequently seen in regions of bone contusion, but do not result in cortical disruption or deformity.
- Appearance: Subchondral linear hypointensity without associated contour deformity or visible involvement of the articular surface.
✅ Osteochondral Fracture
- Definition: Fracture line extending to subchondral bone plate with disruption of articular cartilage.
- Characteristics: Involves both articular cartilage and the subchondral bone plate. Appears as a fracture line that violates the joint surface, subchondral bone plate depression, articular surface disruption and fragmentation, or a combination of these features.
✅ Osteochondral Fracture Types
- Impaction Fracture: Specific type such as deep lateral femoral sulcus occurring after anterior cruciate ligament tear.
- Osteochondral Fragment: Formation of an osteochondral fragment, typically in the setting of acute trauma, involving subchondral bone depression or fragmentation.
