📌MRI Finding of Fracture Line

✅ Acute Traumatic Condition

  • In the acute setting, the fracture line is best shown on T1-weighted MR images as a linear hypointensity.
  • The fracture line may be less conspicuous on T2-weighted images when it is hyperintense and surrounded by bone marrow edema. 
  • However, if there is a component of trabecular impaction, the fracture appears hypointense on both T1- and T2-weighted MR images, similar to the appearance of stress fractures.

 

✅ Trabecular Impaction and Stress Fractures

  • Trabecular impaction causes the fracture to be hypointense on both T1- and T2-weighted MR images. This appearance is similar to stress fractures.

 

Summary of Fracture Line Appearance

  • T2-weighted images: Hyperintense fracture surrounded by bone marrow edema, making it less conspicuous unless trabecular impaction is present, which makes the fracture hypointense on both T1 and T2 images.
