📌 Fosbury Flop Tear

 

✅  Definition and Mechanism
The Fosbury flop tear is a recently described full-thickness tendon tear in which the tendon stump flips upon itself. 

This flipped portion then adheres medially (superomedially on the bursal side), which gives the lesion its characteristic appearance.

An abnormally thickened supraspinatus tendon on the bursal side should raise suspicion for a Fosbury flop tear. 

Radiologists should consider this diagnosis, especially when the characteristic superomedial orientation and accompanying bursal changes are present.

 

MRI Findings

  • Thickened Tendon Stump: On MRI, the tendon stump is abnormally thicker on the bursal side.
  • Orientation: The stump typically shows a superomedial orientation.
  • Additional Signs: There may be fluid accumulation in the superomedial part of the subacromial bursa along with adhesions between the bursal wall and the supraspinatus tendon.

 

Clinical Prevalence and Diagnostic Criteria

  • Prevalence: In a study of 423 patients, 11 cases (approximately 2.6%) were identified as having a Fosbury flop tear.
  • Diagnostic Reliability: The interobserver agreement for identifying key imaging features (such as tendon thickness, orientation, and adhesions) is excellent.
  • : The most sensitive and specific indicator for diagnosing this tear is the abnormal thickness of the tendon stump on the bursal side.
