Anteater Sign

  • Definition: Elongated, squared anterior process of the calcaneus resembling an anteater’s snout as it approaches the navicular.

  • View: Best seen on lateral and oblique radiographs.

  • Diagnostic Value: Specific (≈90–94%) but variably sensitive (10–72%). Absence does not exclude coalition.

  • Pearl: Normal anterior process is short, pointed, triangular; elongation suggests coalition.

Reverse Anteater Sign (Elongated Navicular Sign)

  • Definition: Lateral/posterior extension of the navicular bone, with its lateral AP dimension smaller than the medial.

  • View: Best seen on AP or oblique radiographs.

  • Diagnostic Value: Lower sensitivity (18–50%) but high specificity (≈100%).

  • Pearl: Indicates abnormal navicular morphology contributing to coalition.
