Mild Tendinosis — Key Imaging Features (No Tear)
- Increased intrasubstance signal
(↑ T2 / PD signal, but not fluid-like) - Mild tendon thickening
- Preserved fiber continuity
- No fluid cleft
- No fiber attenuation or discontinuity
- ± Minimal peritendinous or bicipitoradial bursal fluid
- ± Enthesopathic change at the insertion
👉 Important point:
This tendon is not normal, but it is not torn.
Tendinosis vs Tear — Practical MRI Distinction
- Tendinosis
- Tear
Clinical Meaning
- Reflects chronic overuse and degenerative change
- Most cases → conservative treatment
- However:
Persistent degeneration may progress to partial or complete tear - Early recognition helps risk stratification and follow-up planning
