(Short Head vs Long Head — Anatomy & Function)
1. Basic Structure
- The biceps brachii is a two-headed muscle
- The distal biceps tendon is formed by contributions from the
short head (SHB) and long head (LHB) - It is a flat, extrasynovial structure
- Tendon formation begins approximately 7 cm proximal to the elbow
2. Origin of Each Head
- Short head
- Long head
3. Distal Course and Insertion Pattern
- The two heads gradually coalesce as they course distally
- The tendon undergoes an approximate 90° rotation distally
- Short head
- Long head
4. Functional Differentiation (Key Point)
- Short head
- Long head
5. Lacertus Fibrosus (Bicipital Aponeurosis)
- Formed by superficial fibers of the distal biceps tendon
- Fans medially and blends with the pronator teres fascia
- Contributes to elbow flexion and forearm supination
๐ SNS Take-Home Message
- Short head — medial course, distal insertion, flexion dominant
- Long head — lateral course, proximal insertion, supination dominant
