Key Teaching Point

The oblique meniscomeniscal ligament (OMML) is a normal intermeniscal ligament variant that can closely mimic a bucket-handle meniscal tear if not carefully evaluated.

What It Is

  • A type of intermeniscal ligament
  • Reported prevalence: 1–4%
  • Traverses the intercondylar notch, passing between the ACL and PCL

How It’s Named

Oblique meniscomeniscal ligaments are named by their anterior attachment site:

  • Medial OMML:
    Anterior horn of the medial meniscus → posterior horn of the lateral meniscus
  • Lateral OMML:
    Anterior horn of the lateral meniscus → posterior horn of the medial meniscus

Why It Matters

  • Has signal intensity similar to meniscal tissue
  • On sagittal images, it may resemble:
  • This can lead to overcalling pathology

How to Differentiate from a Bucket-Handle Tear

  • Follow the structure on sequential coronal images
  • Identify continuity from anterior horn to opposite posterior horn
  • Confirm normal meniscal body size and shape
