Imaging Signs of Isolated Posterolateral Bundle Tear of the ACL

An ACL tear may only involve one bundle. The specific imaging signs of an isolated posterolateral bundle tear include:

1️⃣Gap Sign

  • Description: Presence of fluid signal and/or a gap.
  • Location: Between the medial aspect of the lateral femoral condyle and the lateral aspect of the mid-ACL.
  • Imaging Modalities: Can be seen on either axial or coronal MRI images.

2️⃣Footprint Sign

  • Description: Incomplete coverage of the lateral aspect of the tibial spine of the tibia by the distal ACL attachment.
  • Location: Specifically observed on the tibial spine.
  • Imaging Modalities: Seen only on coronal MRI images.
