📌 Complete Tear of the Medial Retinaculum
✅ Case Study
- In axial images, a complete tear of the Medial Patellofemoral Ligament (MPFL) can be easily identified, accompanied by lateral patellar subluxation.
- However, it's not just the MPFL that is torn at the patellar side – the medial patellomeniscal ligament and the medial patellotibial ligament within the MPFL complex are also involved.
✅ Progression of the Tear
- Initially, partial ruptures of the medial retinaculum occur at the inferomedial patella.
- This tear can then extend proximally, with the retinacular/ligamentous injury starting inferiorly and progressing superiorly.
- In complete tears, this injury extends all the way to the region typically defined as the MPFL patellar insertion.
✅ Structures Involved
- The medial retinaculum encompasses both the MPFL and the Medial Patellotibial Ligament (MPTL).
- Therefore, both of these structures are affected in a complete tear.
- What starts as a partial tear gradually extends proximally, resulting in a complete tear of the MPFL and MPTL.
