📌 Poor Outcome of Osteochondral Autograft Transplantation (OATS)

 

✅ Signs of Poor Tissue Integration

  • Persistent edema-like marrow changes within subchondral bone beyond 18 months
  • Subchondral cyst formation

 

✅ Depression and Proud Grafts

  • Depression of the plug less than 1 mm can still result in good clinical outcomes
  • Proud grafts can increase contact pressure, leading to degenerative changes

 

✅ Graft-Host Interface Fissures

  • Fissures at the graft-host interface indicate poor outcomes
  • Histologically, transplanted mature articular cartilage does not incorporate into adjacent cartilage

 

✅ Monitoring Subchondral Cysts and Bone Marrow Edema (BME)

  • Formation of subchondral cysts
  • Fluid-intense rim around the osseous part of the graft
  • Persistent subchondral BME or newly developed BME may indicate poor graft incorporation
  • Osteonecrosis is a rare complication of OATS
