📌 Distal Clavicle Osteolysis
1. Overview
Distal clavicle osteolysis is a painful bone resorption disorder occurring in two forms:
- Posttraumatic: Develops months to years after an acute injury.
- Overuse: Stress-induced, common in weightlifters and throwing athletes.
Both forms show similar histopathologic features, including synovial inflammation, fibrosis, cartilage degeneration, and subchondral microfractures.
2. Epidemiology & Symptoms
- Primarily affects young adult males.
- Symptoms include joint pain, pain radiating to the arm/neck, and difficulty sleeping on the affected side.
- 20% of cases are bilateral.
3. Imaging Findings
- Radiographs: Late-stage cortical irregularity, flame-shaped resorption, subchondral cysts.
- MRI: Early intense clavicular edema, effusion, capsular swelling, and subchondral cysts.
- CT & Ultrasound: Useful for distinguishing from other conditions.
4. Differential Diagnoses
- Septic Arthritis: Rare, often in immunocompromised individuals; requires early aspiration and surgical intervention.
- Rheumatoid Arthritis: Chronic inflammatory disease leading to AC joint destruction.
- Hyperparathyroidism: Systemic bone resorption, often bilateral.
- Neoplasms & Gorham Disease: Rare but can cause clavicular osteolysis.
5. Summary
Distal clavicle osteolysis primarily affects young, active males, often due to repetitive stress or past trauma. MRI is the most sensitive imaging tool. Differential diagnoses include infections, systemic diseases, and rare disorders.
