  • Osteoarthritis-like changes in atypical joints
  • Symmetric distribution
  • Non–weight-bearing joint predominance
  • Frequent involvement of fibrocartilage and ligaments
  • Calcification within cartilage or fibrocartilage (not on the surface)

 

2. Radiography / CT (Key Diagnostic Modalities)

  • Chondrocalcinosis
  • Joint space narrowing
  • Osteophytes
  • Subchondral changes
  • CT

 

Wrist & Hand (Classic CPPD Pattern)

  • Radiocarpal and scapholunate joints
  • TFCC chondrocalcinosis
  • Lunotriquetral ligament calcification
  • Stepladder pattern of joint space narrowing
  • STT (scaphotrapeziotrapezoid) arthropathy
  • Advanced cases:

 

Knee (Most Common Site)

  • Medial meniscus chondrocalcinosis
  • Patellofemoral joint involvement
  • Anterior femoral cortex scalloping at patellar level
  • Osteoarthritis pattern disproportionate to age or loading

Radiologist’s Take-Home Message

CPPD arthropathy is a pattern diagnosis.
Calcification location, joint selection, and symmetry matter more than crystal visibility alone.
