[Anatomy_20] Extensor carpi ulnaris (ECU) subsheath complex, Linea Jugata

ECU subsheath connected the ulnar styloid process and superficial ligamentous fibers of the dorsal radioulnar ligament.

So, The subsheath of the ECU tendon maintains the stability of the dorsal aspect of the TFCC 

it is necessary to learn about the ECU subsheath.

 

The extensor retinaculum is an important and complex structure, preventing extensor tendons bowstringing.

 

The extensor retinaculum courses over the ECU and distal ulna to attach to the pisiform and triquetrum, inserts into the sheath of the flexor carpi ulnaris tendon, and blends with the volar antebrachial fascia.

ER does not stabilize the ECU tendon.

 

There was a thin membranous structure of ECU subsheath consistently lying below the extensor retinaculum and holding the ECU tendon in the ulnar groove

A unique anatomical characteristic of the ECU is the fibro-osseous tunnel which stabilizes the tendon at the level of the distal ulna.

This fibro-osseous tunnel is formed by the distal ulna and a 1.5 to 2cm in length band of connective tissue referred to as the ECU subsheath.

 

The subsheath lies deep to the extensor retinaculum,which itself does not attach to or stabilize the ECU tendon.

The ECU,its subsheath,and the extensor retinaculum are readily seen using MRl.



The ECU subsheath attached to the underlying periosteum at the margins of the ulnar groove

Schematic drawing shows the unique anatomy of the radial and ulnar attachments of the ECU subsheath. 

Unlike the anatomy of the radial leaf insertion of the subsheath, the ulnar aspect is almost pyramidal in cross-section

The radial side shows a limbus attachment, while the ulnar side shows a labral attachment

The longitudinal fibers of the linea jugata are differentiated from radial side

The linea jugata is longitudinal fibers that reinforced the ulnar insertion of the subsheath
