1️⃣ Graft Preparation:
The medial meniscus graft is readied with bone plugs attached to each horn, reflecting the anatomical distance between the horns and their medial orientation relative to the ACL insertion.
2️⃣ Bony Sockets Creation:
In the recipient's proximal tibia, two sockets are crafted at the sites of
the original meniscal roots to accommodate the anterior and posterior bone plugs from the donor tissue.
3️⃣ Bone Tunnels:
Bone tunnels are established from each socket to the anterior tibial cortex. These tunnels enable the sutures attached to the bone plugs to be pulled through to the front of the tibia.
4️⃣ Suture Management:
The anterior and posterior pull-in sutures, attached to the bone plugs, are drawn out through the bone tunnels. They are then securely tied to each other, ensuring that both bone plugs are snugly positioned within the proximal tibia.
5️⃣ Final Steps:
To finalize the transplantation, the allograft meniscus is sutured to the surrounding joint capsule, completing the intricate process of medial MAT using the double bone plugs technique.
