Post-PCL Reconstruction Complications
① Recurrent instability
- Recurrent instability is a common complication, possibly due to graft tear/stretching, impingement, or malpositioned tunnels.
- Graft tears may occur due to new injury or chronic impingement from suboptimal tunnel positioning.
- MRI signs of graft tear are similar to ACL but have lower sensitivity.
- Posterior tibial translation ≥3.6 mm may indicate graft failure.
② Arthrofibrosis
- Arthrofibrosis is much more frequent after PCL reconstruction compared to ACL reconstruction.
- Some degree of arthrofibrosis may contribute to knee stabilization and be considered normal if the range of motion is not limited.
- Diffuse arthrofibrosis appears as ill-defined, low-signal spiculated areas within the Hoffa fat pad, around the graft, extending to the joint capsule and causing synovial/capsular thickening.
