✅ Double-Bundle PCL Reconstruction

  • The double-bundle technique aims better to mimic native PCL for improved posterior stability in flexion
  • Biomechanical studies show superior knee laxity and kinematics restoration compared to single-bundle

  • Potential for excessively restricted residual anteroposterior translation
  • No significant differences in clinical, radiological, and survivorship outcomes at ≥10 years follow-up

①. Femoral Tunnels

  • Anterior Bundle: Anterior 1/3 of native PCL insertion

Right knee: 1 o'clock, Left knee: 11 o'clock

  • Posterior Bundle: Middle to distal 1/3

Right knee: 3 o'clock, Left knee: 9 o'clock

②. Tibial Tunnel

  • Placement varies between single and double-bundle techniques
  • Avoid anterior placement to prevent injury to the anterior medial meniscal root
