✅  Anatomical Location and Structure

  • Capsular ligament located in the deepest (third) layer of the lateral aspect of the knee
  • Femoral origin is located posterior and proximal to the lateral collateral ligament (LCL)

✅  Visibility on Imaging

  • Variable visibility, ranging from 51% to 100% of cases
  • Distal tibial portion is the most consistently visualized, often best depicted on coronal sequences

✅  Associated Injuries

  • Closely associated with concomitant anterior cruciate ligament (ACL) tear
  • Higher likelihood of Segond fracture or distal avulsion near the lateral tibial plateau

✅  Accuracy of MRI in Diagnosis

  • Accuracy as high as 88% for identifying ALL injury in the setting of concurrent ACL injury
  • Sensitivity may decrease for subacute (>1 month) and chronic injuries

✅  Other Associated Injuries

  • ALL injury is more likely to be associated with lateral meniscal tear, collateral ligament injury, and osseous injury
