📌Overview of the Anterolateral Ligament (ALL)
The Anterolateral Ligament (ALL) is a capsular ligament located in the deepest lateral layer of the knee, deep to the iliotibial band (ITB).
It has an oblique anterior and inferior course, inserting on the lateral meniscus and lateral tibial condyle.
✅ ALL Insertions
The ALL has 3 main insertions: femoral, tibial, and meniscal. On coronal imaging, these cannot all be visualized at once.
1️⃣ Tibial Insertion
- Varied the least among studies
- Located midway between the fibular head and Gerdy's tubercle, 4-7mm below the tibial plateau
- Posterior ITB fibers and anterior ALL fibers are indistinguishable, both inserting on Segond fractures in 94% of cases
2️⃣ Meniscal Insertion
- Located between the body and anterior horn of lateral meniscus
- Lateral inferior geniculate artery is a useful landmark, with bifurcation seen leading to meniscal and tibial insertions
- Three common insertion shapes: inferior-only, complete, bipolar
3️⃣ Femoral Origin
- Exact location debated, but current understanding is posterior and proximal to lateral collateral ligament (LCL)
- Difficult to differentiate from LCL and ITB due to tight confluence at lateral epicondyle
