📌 Trapped Periosteum in Pediatric Proximal Tibial Physeal Injuries

  •  MRI guides surgical planning when conventional reduction fails, potentially improving outcomes.

 

✅ MRI Findings of Trapped Periosteum:

  • widening of proximal tibial physis
  • Enfolded periosteum in widened tibial physis
  • Low signal intensity of trapped periosteum in growth plate

 

✅Benefits of MRI:

  1. Evaluates menisci, ligaments, and articular cartilage
  2. Assesses non-ossified epiphyses
  3. Detects osteonecrosis and bony bridging
  4. Identifies mechanical obstructions to the reduction
  5. Visualizes periarticular structures (nerves, vessels, tendons, muscles)

 


✅ Type IV Fractures

  • Type IV fractures present with fracture extension from the tibial tuberosity through the proximal tibial physis into the posterior tibial metaphyseal cortex.
