📌 Case 1: Tibial Tuberosity Fracture

  • Radiograph: Minimally displaced fracture
  • MRI: Distal tibial tubercle fracture with minimal separation
  • Edema at fracture site, surrounding tissues, and Hoffa's fat pad
  • Ogden classification: Type 1A

✅ Type I Fractures

Type I fractures involve only the distal portion of the tubercle.

  • Type IA: Minimally displaced or non-displaced fracture.
  • Type IB: Anterior and proximal displacement or comminution.
  • Type IC: Proposed subtype for associated patellar tendon avulsions.

 

📌 Case 2: Cartilaginous Stage Osgood-Schlatter Disease

  • Radiograph: May appear normal
  • MRI: Incomplete transverse clefts in tuberosity's secondary ossification center
  • Mild proximal tibial marrow edema and avulsed secondary ossification center

 

✅ Similarities:

  • Proximal tibial marrow edema
  • Secondary ossification center changes
  • Patellar tendon thickening and signal alteration
  • Deep infrapatellar bursitis and Hoffa's fat pad edema

 

✅ Key Differences:

  • Fracture: Fracture line through tubercle ossification center
  • Osgood-Schlatter: Transverse clefts in secondary ossification center
