📌 ICRS Cartilage Defect Classification
✅ ICRS Grade 3
- Cartilage defects extending to more than 50% of cartilage depth but not through the subchondral bone.
- Includes chondral blisters, delamination, and chondral flaps without associated subchondral marrow signal.
★ Subcategories:
- ICRS Grade 3A: Defect more than 50% but not down to the calcified layer.
- ICRS Grade 3B: Defect more than 50% down to the calcified layer.
- ICRS Grade 3C: Defect down to but not through the subchondral bone plate.
- ICRS Grade 3D: Defect more than 50% with blisters.
✅ ICRS Grade 4
- Full-thickness chondral lesions extending to the subchondral bone plate with subchondral marrow edema-like signal or cystic changes.
- When the cartilage lesion itself does not clearly extend to the subchondral bone, full-thickness extent is suggested by subchondral cysts or bone marrow edema.
★ Subcategories:
- ICRS Grade 4A: Defect includes superficial subchondral bone plate.
- ICRS Grade 4B: Defect down to deep subchondral bone.
