📌 Subchondral Fracture with Epiphyseal Collapse: deformity of the subchondral bone plate
- In the previous session, we discussed various MRI findings of SFI, categorizing them into two groups: those without epiphyseal collapse and those with epiphyseal collapse.
- This session will focus on cases showing epiphyseal collapse along with deformity of the subchondral bone plate.
- Three major imaging findings are observed in these cases:
Subtle Flattening of the Articular Surface
Subchondral Area of Low Signal Intensity
Subchondral Hypointense Line

- It is important to understand that the terms “epiphyseal collapse” and “articular collapse” are both acceptable as descriptive terms but are not specific to any single condition.
- Collapse can occur in osteonecrosis, SIF, and rapidly destructive osteoarthritis.
- The term “collapse” should be used to describe a fracture of the subchondral bone plate occurring in an atraumatic setting. For acute trauma, the preferred term is “osteochondral fracture.”
