📌 Extracapsular Tophi

Extracapsular tophi typically affect the prepatellar bursae and extensor mechanism, leading to several characteristic changes:

  • Anterior soft-tissue prominence
  • Bursal distention
  • Tendon enlargement due to intratendinous urate deposition
  • Well-marginated erosions at the patella and tibial tuberosity

📌 Subcutaneous or Extra-Articular Tophi

  • Subcutaneous or extra-articular tophi are usually a late manifestation and often occur together with intra-articular tophaceous deposition.
  • Extra-articular tophi are relatively rarer than intra-articular tophi, and in most patients, extra-articular tophi are associated with intra-articular lesions.
