1. Etiology and Risk Factors

Acromial apophysiolysis is theorized to result from repetitive stress, particularly overhead movements seen in activities like baseball pitching and swimming.

 

2. MRI Characteristics of Acromial Apophysiolysis

Key MRI findings include:

  • A transverse or linear interface between the primary bone plate and secondary ossification centers.
  • An irregular, widened, or fluid-containing interface.
  • Marrow and soft-tissue edema adjacent to the affected area.
    These signs may indicate instability and could be an early marker for os acromiale formation.

 

3. Clinical Significance of MRI Findings in Adolescents

MRI findings consistent with os acromiale in adolescents should not be dismissed as normal development. If transverse interface orientation, irregular margins, and edema are present, early intervention should be considered to prevent long-term complications such as rotator cuff pathology.

 

4. Prevalence and Fusion Timeline

The reported prevalence of os acromiale ranges from 1% to 30%. 

Traditionally, it was believed that fusion could occur up to 25 years of age, with imaging-based diagnosis reserved for adults. 

However, recent literature suggests that most children achieve fusion by 16–17 years of age.
