Peroneus longus
- Inserts on 1st metatarsal base & medial cuneiform.
- Everts, plantarflexes, stabilizes transverse arch.
- Rarely harvested due to key role in foot stability.
Peroneus brevis
- Inserts on 5th metatarsal base.
- Strong evertor.
- Common donor for Achilles reconstruction.
Achilles tendon
- Largest tendon (gastrocnemius + soleus).
- Inserts on calcaneus, provides powerful plantarflexion.
- Surrounded by paratenon, not a synovial sheath.
Plantaris tendon
- Small, variable, runs medial to Achilles.
- Functionally insignificant but valuable graft source.
Clinical relevance
- Peroneus brevis & plantaris frequently used for Achilles repair.
- Anatomy critical for MRI interpretation and surgical planning.
