Peroneus longus

  • Inserts on 1st metatarsal base & medial cuneiform.

  • Everts, plantarflexes, stabilizes transverse arch.

  • Rarely harvested due to key role in foot stability.

Peroneus brevis

  • Inserts on 5th metatarsal base.

  • Strong evertor.

  • Common donor for Achilles reconstruction.

Achilles tendon

  • Largest tendon (gastrocnemius + soleus).

  • Inserts on calcaneus, provides powerful plantarflexion.

  • Surrounded by paratenon, not a synovial sheath.

Plantaris tendon

  • Small, variable, runs medial to Achilles.

  • Functionally insignificant but valuable graft source.

Clinical relevance

  • Peroneus brevis & plantaris frequently used for Achilles repair.

  • Anatomy critical for MRI interpretation and surgical planning.
