Flap Tear vs. Bucket-Handle Tear:

  • Flap tears, unlike bucket-handle tears, do not show complete posterior-to-anterior extension and displacement of the involved meniscal fragment. In this scenario, the flap tear mimics a bucket-handle tear, yet distinct differences are evident upon closer examination.

Bucket-Handle Meniscal Tears:

  • These are a type of displaced vertical meniscal tear where the inner part is displaced centrally,
  • resulting in the central migration of the inner “handle” fragment.

The Double Posterior Cruciate Ligament (PCL) Sign:

  • This sign results from meniscal material in the notch, inferior and parallel to the PCL in the same sagittal plane. 
  • The displaced fragment, in proximity and posterior to the anterior horn segment, contributes to this sign.

The Anterior and Posterior Triangles:

  • In this context, the anterior triangle represents the normal anterior horn, while the posterior triangle symbolizes the displaced bucket handle segment.

This case exemplifies the need for a meticulous approach in MRI interpretation to distinguish between similar yet distinct meniscal tear patterns.
