Horizontal Flap Tear
- In a horizontal flap tear, we observe a meniscal fragment displaced deeper than the Medial Collateral Ligament (MCL). However, in a horizontal cleavage tear, there is no meniscal fragment situated deeper than the MCL.
- ๐๏ธ๐จ๏ธ Diagnosing a horizontal flap tear can be challenging and is sometimes overlooked. Given that most displaced meniscal tears are found medially and posterolaterally in the medial meniscus, it's crucial to scrutinize these areas closely.
Horizontal Cleavage Tear
- Horizontal meniscal tears occur within the meniscus in a horizontal plane, dissecting through its circumferential collagen fibers.
- They appear as horizontally oriented lines with increased intrameniscal signal, often extending to either the superior or inferior surface of the meniscus, typically near the free edge.
- Most horizontal tears reach the meniscus's inferior surface.
- ๐ When a horizontal tear extends to the meniscal apex, it's referred to as a pure horizontal cleavage tear.
