Normal ACL on Sagittal Images
- Signal Intensity: The normal Anterior Cruciate Ligament (ACL) exhibits low signal intensity on both T1- and T2-weighted MRI sequences. Heterogeneous striations, which are commonly observed, are usually due to the presence of fat and synovium, especially at the tibial attachment.
- Sagittal MR Images with Extended Knee: When MR images are obtained with the knee extended, the ACL fibers appear taut. They run parallel to or within 9° of the Blumensaat line, which is the roof of the intercondylar notch.
- Anatomical Considerations: The anterior margin of the ACL may appear darker and thicker than the remainder of the ligament. This characteristic corresponds anatomically to the ACL's anteromedial bundle (AMB).
- Knee Flexion: The ACL may demonstrate a more lax configuration with knee flexion. Biomechanically, the AMB tightens during this motion while the Posterolateral Bundle (PLB) loosens.
