★ ACL Tibial Landmarks ★

1️⃣ ACL Tibial Attachment

  • The ACL's Anteromedial (AM) bundle footprint presents a convex, comma-shaped appearance at the tibial attachment. This shape encompasses the anterior, convex contour of the Posterolateral (PL) bundle, connecting anterolaterally (via the tail of the comma) and anteromedially (via the head of the comma).

2️⃣ Lateral Meniscal Attachments to ACL

  • The AM bundle is aligned with the anterior horn of the lateral meniscus.
  • The anterior aspect of this horn aligns with the center of the AM bundle, while its posterior aspect aligns with the center of the PL bundle.
  • The midpoint along the medial attachment border of the anterior horn of the lateral meniscus aligns with the center of the ACL.

3️⃣ ACL Ridge and Tubercle

  • A bony elevation runs between the anterolateral fossa and the medial border of the tibial plateau articular cartilage.
  • This elevation marks the anterior-most border of the ACL tibial attachment and is termed the ACL ridge.

4️⃣ Anterolateral Fossa

  • It is identified as a bony depression located immediately medial to the lateral tibial plateau articular cartilage border and anterior to the lateral tibial eminence.
  • The posterior aspect of the anterolateral fossa serves as a bony reference located posterolateral to the overall ACL attachment center and corresponds to the attachment of the anterior horn of the lateral meniscus.

5️⃣ Retro-eminence Ridge

  • Also known as the "over the back" ridge or the "PCL notch," this transverse intereminence ridge is found at the apex of the posterior slope of the tibial plateau, just anterosuperior to the tibial PCL attachment.
