✅Understanding ACL Stump Entrapment: Diagnosis and Management ✅

  • ACL stump entrapment (SE) is a condition where a fragment of the torn anterior cruciate ligament (ACL) gets displaced into the intercondylar notch, potentially causing knee extension limitations or locking. 
  • This displacement can trigger inflammation and the accumulation of fibrous tissue, leading to discomfort and restricted movement.
  • The Significance of Timely Diagnosis Promptly identifying SE is crucial for determining the optimal timing for surgical intervention. 

▶️▶️ Surgical Considerations and Timing: 

  • Early surgical reconstruction is advisable for patients whose mechanical block caused by SE is unlikely to be resolved through physical therapy. 
  • Early removal of the entrapped stump facilitates better outcomes for ACL reconstruction by preventing further complications and promoting recovery.

▶️▶️ Preserving ACL Stumps: 

  • A Sensory Perspective Considering the proprioceptive qualities of the residual ACL stumps, preserving them as much as possible is beneficial. 
  • These stumps not only retain sensory functions but also may contribute to the re-innervation of the newly implanted ACL graft, helping to avoid complete sensory loss in the affected area.
