1️⃣PCL Injuries Overview

  • Multiligamentous injuries involving the PCL are more common than isolated PCL tears. Midsubstance tears are the most prevalent form. The PCL can be injured through its substance or at its tibial and femoral attachments.

2️⃣ Surgical Management

  • Proximal third PCL tears and those involving multiple knee soft tissue structures are more likely to be treated surgically than conservatively. Precise reporting of PCL tear location, grade, and associated injuries on knee MRI is crucial for guiding treatment.

3️⃣"Peel-off" PCL Tears

  • Complete PCL tears from its femoral attachment without bony avulsion, known as "peel-off" tears, may be repaired arthroscopically. These femoral origin tears can appear subtle, with mild edema and no significant gap.
