1️⃣ Mucoid Degeneration of the PCL

  • Diagnosis Criteria: Mucoid degeneration of the PCL should be diagnosed when the ligament is intact and thickened, with increased signal intensity on fluid-sensitive sequences. This is especially relevant in patients with no history of trauma or knee injury and no clinical signs of ligamentous instability.

  • Characteristic Appearance: Intrasubstance mucoid degeneration of the PCL can demonstrate a striated “tram-track” appearance with thickening that might mimic a tear.

2️⃣ Mucoid Degeneration vs. Partial Tear

  • Differentiation: Mucoid degeneration may be confused for a PCL injury, but the normal course of fibers along with the presence of low-signal-intensity margins are key distinguishing features.

  • Key Observation: In intrasubstance mucoid degeneration of the PCL, which results in a “tram-track” appearance, the abnormal signal should not violate the ligament's margins.
