📌 Overview of the Posteromedial Corner
The posteromedial corner is defined by the area between the posterior margin of the superficial medial collateral ligament (sMCL) and the medial border of the posterior cruciate ligament (PCL).
1️⃣ Posterior Oblique Ligament (POL)
The POL's meniscal attachment, known as the POL capsular arm, is an extension of the posteromedial capsule. It is strategically positioned between the posterior meniscocapsular attachment and the meniscofemoral attachment.
2️⃣ Meniscocapsular and Meniscotibial Ligaments
Situated more posteriorly, the medial meniscus's posterior horn is supported by:
- The Meniscocapsular Ligament, which extends posterosuperiorly to the capsule (20mm).
- The Meniscotibial Ligament, attaching inferiorly to the tibia (14mm).
- The Medial Meniscus itself measures 21mm from medial to lateral.
3️⃣ Semimembranosus Expansions
The semimembranosus tendon, a key component of the posteromedial corner, comprises two main parts:
- The Direct Arm: Inserts at the proximal tibia.
- The Anterior Arm: Extends along the proximal tibia, inserting deep or posterior to the sMCL, connecting to the coronary ligament at the medial meniscus,
✅Special Note on Semimembranosus-Meniscal Attachment
The semimembranosus-meniscal attachment originates from the anterior arm of the semimembranosus, located between the posterior meniscotibial ligament and the meniscotibial attachments of the POL.
This connection emphasizes the importance of the anterior arm, which passes under the POL and attaches to the coronary ligament at the medial meniscus's posterior horn.
