📌 Meniscotibial Ligament and Its Importance

The meniscotibial ligament plays a crucial role in maintaining the stability and position of the meniscus. 

Its tibial attachment is located on the medial aspect of the medial rim of the tibial plateau, near the joint line and proximal to the attachment of the anterior arm of the semimembranosus expansion.

✅ Consequences of Meniscotibial Ligament Rupture

  • Rupture of the meniscotibial ligament may contribute to instability of the meniscus, as this ligament normally adheres the meniscus tightly to the tibia and prevents extrusion.
  • Disruption of the deep medial collateral ligament (dMCL) may clinically mimic a meniscal tear, highlighting the importance of careful examination.

✅ Meniscus Extrusion and Root Tears

  • Disruption of the meniscotibial ligament and subsequent meniscus extrusion may increase stress at the root, potentially causing a root tear.
  • Emerging evidence suggests that meniscus extrusion can also cause root tears, not just the other way around.
  • Serial imaging examinations have shown that meniscotibial ligament disruption and meniscus extrusion seen on MRI may precede and contribute to the development of root tears due to increased stresses at the root.
  • There is growing interest in surgical reduction (centralization) of extruded menisci, along with repair of the meniscotibial ligament, to restore the normal anatomy and biomechanics of the meniscus and potentially prevent or delay the progression of meniscal tears.
