📌Iliotibial Band (ITB) Injury in Acute Knee Trauma

 

✅ Association with ACL Tear and Patellar Dislocation

  • In up to two-thirds of cases of IT tract injury, the ACL will also be torn. In up to 20% of cases, patellar dislocation will also have occurred. 
  • Injury to the ITB is usually an indicator of significant internal derangement, with a strong association with ACL rupture, significant posterolateral corner injury, and acute patellar dislocation.

✅ Avulsion Fracture at Gerdy's Tubercle

  • The iliotibial band inserts on the lateral tibia at the Gerdy tubercle, and although rarely injured, both soft-tissue and osseous avulsions have been reported with a complex PLC injury and concomitant ACL lesions. 
  • Segond fractures and avulsion of the Gerdy tubercle also have been associated with PLC injuries.

✅ Insertional Tendinosis and Peritendonitis

  • Unlike Iliotibial band syndrome (ITBS), insertional tendinosis and peritendonitis are not related to chronic compression or friction. 
  • The pathogenesis of insertional tendinosis usually involves aging and overuse, particularly in unstable joints due to osteoarthrosis. 
  • MRI may demonstrate thickening and increased intrasubstance intermediate signal intensity, most prominent on fluid-sensitive sequences.
