📌 Components of the MPFL Complex (MQTFL, tMPFL,odMPFL)

 

1️⃣ Adductor Tubercle Level

  • Medial Quadriceps Tendon Femoral Ligament: At the proximal level of the adductor tubercle's insertion, this component of the MPFL complex can be identified.

 

2️⃣ Distal to the Adductor Tubercle Level

  • Transverse Component of the MPFL: Slightly distal to the adductor tubercle, the transverse component of the MPFL is seen, attaching below the tubercle and above the superficial medial collateral ligament (sMCL).

 

3️⃣ Proximal Superficial MCL Level

  • Oblique Decussation Component: More distally, the oblique decussation component of the MPFL attaches to the proximal fibers of the sMCL, arising from the anterior margin of the upper sMCL fibers.

 

📌 Relationship with the Superficial MCL

  • The sMCL attaches proximal and posterior to the medial epicondyle. In cases of sMCL tears, the femoral third of the oblique decussation MPFL component (arising from the anterior upper sMCL) is abnormal in approximately 90% of cases, despite no evidence of lateral patellar dislocation on initial MRI.
