📌 Genu Valgum and Rotational Malalignment
- Lower extremity imaging is essential in evaluating genu valgum and rotational malalignment, such as excessive femoral anteversion and external tibial rotation.
1️⃣ Femoral Anteversion:
- Definition: The angle between the posterior condyle line and the femoral neck axis.
- Clinical Impact: Increased femoral anteversion causes medialization of the trochlea, creating a lateral vector force on the patella, leading to abnormal patellofemoral load and lateral subluxation.
- Measurement: Typically, values greater than 30° on 3D CT indicate excessive femoral anteversion.
- Management: Derotational osteotomy is recommended for significant rotational deformity.
✅ Considerations:
- Age Dependency: Femoral anteversion decreases from birth (30-40°) to adulthood (8-14°). Males usually have slightly less anteversion compared to females.
2️⃣ Tibial Torsion:
- Impact: Increased external tibial torsion leads to foot external rotation and knee valgus, contributing to a lateral force on the patella.
