📌Osteochondral Defect at the Medial Margin of the Patella

  • Osteochondral injuries associated with transient lateral patellar dislocation typically present with well-defined, full-thickness, or large partial-thickness defects with acutely angled margins.
  • It is crucial to search for loose chondral or osteochondral fragments within the joint to prevent further complications.

 

✅ Displaced Osteochondral Fragment within the Lateral Recess

  • A slab of articular cartilage and a sliver of the subchondral bone plate can be observed.
  • There is a large displaced osteochondral fragment at the lateral aspect of the lateral femoral condyle and a bone bruise at the anterolateral aspect of the lateral femoral condyle.

 

Significance of the Radiologist's Diagnosis

  • The radiologist’s diagnosis is crucial because if the lesions are larger than about one square centimeter, prompt surgery is indicated.
  • Ideally, surgery should be performed within one week of the event.

 

✅ Osteochondral Defect of the Lateral Edge of the Condyle

  • This case shows an osteochondral defect at the lateral edge of the condyle, requiring careful evaluation for appropriate surgical intervention.

 

✅ Complete Tear of the Patellar Side of the Medial Patellofemoral Ligament (MPFL)
