📌 Three Types of Medial Patellofemoral Ligament (MPFL) Avulsion Injuries at the Patella

  • Generally, MPFL ruptures at the midsubstance or near the patellar attachment are treated non-surgically. 
  • However, significant patellar MPFL avulsion fractures and femoral attachment MPFL disruptions may pose greater risks. 
  • Skeletal immaturity can influence the injury pattern, with isolated patellar-based MPFL/chondral injuries being more common in skeletally immature patients.

 

1️⃣ Type P0: 

Ligamentous disruption at the patellar attachment (partial tear of femoral MPFL). 

 

2️⃣ Type P1: 

Bony avulsion fracture from the medial margin of the patella (small avulsion fracture of femoral MPFL), may involve a tear of the transverse component (tMPFL). 

 

3️⃣Type P2: 

Bony avulsion involving articular cartilage from the medial facet of the patella.
