📌 Patellar Tendon-Lateral Femoral Condyle Friction Syndrome
✅ Definition and Causes
- Patellar tendon-lateral femoral condyle friction syndrome is associated with patellar maltracking.
- It commonly affects adolescents and young women, involving impingement between the patellar tendon and the lateral femoral condyle.
- This leads to localized edema in the superolateral Hoffa's fat pad [1].
✅ Clinical Presentation
- Patients typically experience anterior or anterolateral knee pain, which worsens with activities like hyperextension, flexion, and extension [2].
- This syndrome is more prevalent in young individuals engaged in regular physical activities.
✅ MRI Findings
- MRI often reveals abnormal signal intensity consistent with edema in the superolateral IPFP.
- Other findings may include a high-riding patella, increased TT-TG distance, and reduced patellar tendon–trochlea distance [3].
- The abnormal signal can extend into the central superior aspect of the fat pad and may enhance with contrast injection [2].
- A minority of patients may exhibit lobular lesions with fluid/cystic signal changes and abnormalities of the adjacent patellar tendon [2].
- Advanced changes related to patellar tracking dysfunction, such as patellar tendon tearing and patellofemoral arthrosis, are also associated with edema in this location [4].
