📌Post-traumatic Lesions of Hoffa’s Fat Pad
✅ Fractures and Shear Injuries
- Fractures near Hoffa’s fat pad, including patellar, tibial, and femoral condylar fractures, often show reactive edematous changes due to shear injury or direct impact.
- Shear injury in Hoffa’s fat pad is also associated with ACL injuries (1).
✅ Acute Injury Associations
- Acute injuries to Hoffa’s fat pad are typically linked to other anterior knee injuries, such as patellar tendon tears, patellar sleeve injuries, patellofemoral dislocations, or patellar fractures.
- Patellar dislocations can cause partial tears at the superomedial border.
- Complete tearing of the fat pad is rare without severe trauma (2).
✅ Inflammation and Arthrofibrosis
- Both acute and chronic traumas can lead to local bleeding and inflammation, the main causes of Hoffa’s fat pad-related pain.
- These injuries can result in arthrofibrotic lesions, such as post-traumatic or post-arthroscopic fibrosis (3).
✅ Hemarthrosis and ACL Tears
- Hemarthrosis from acute ACL tears is characterized by synovitis with an irregular free concave edge of Hoffa's fat pad.
- Shearing injuries to the fat pad can also result in fractures, often seen with ACL tears (4, 5).
