📌 Patellar Tendon-Lateral Femoral Condyle Friction Syndrome

 

✅ Definition and Causes

  • Patellar tendon-lateral femoral condyle friction syndrome is associated with patellar maltracking. 
  • It commonly affects adolescents and young women, involving impingement between the patellar tendon and the lateral femoral condyle. 
  • This leads to localized edema in the superolateral Hoffa's fat pad [1].

 

✅ Clinical Presentation

  • Patients typically experience anterior or anterolateral knee pain, which worsens with activities like hyperextension, flexion, and extension [2]. 
  • This syndrome is more prevalent in young individuals engaged in regular physical activities.

 

✅ MRI Findings

  • MRI often reveals abnormal signal intensity consistent with edema in the superolateral IPFP. 
  • Other findings may include a high-riding patella, increased TT-TG distance, and reduced patellar tendon–trochlea distance [3]. 
  • The abnormal signal can extend into the central superior aspect of the fat pad and may enhance with contrast injection [2]. 
  • A minority of patients may exhibit lobular lesions with fluid/cystic signal changes and abnormalities of the adjacent patellar tendon [2].
  • Advanced changes related to patellar tracking dysfunction, such as patellar tendon tearing and patellofemoral arthrosis, are also associated with edema in this location [4].
