📌 Morel-Lavallée Lesion

✅ Definition and Clinical Presentation

  • A Morel-Lavallée lesion is a closed degloving injury that occurs when there is a shearing force between the subdermal tissues and the underlying fascia. 
  • This creates a potential space that fills with fluid, blood, lymph, and/or necrotic fat. It commonly occurs in the prepatellar region due to falls onto the knee or direct blows during sports activities .
  • Patients with a Morel-Lavallée lesion often present with a soft, painless mass. The lesion can cause palpable fluctuance, limited knee flexion, and decreased sensation.

✅ Imaging Characteristics

  • Acute to Subacute Phase: T2 hyperintense fluid with blood clots and debris within an ovoid cavity. The effusion may appear increased or intermediate in signal intensity on T1-weighted images.
  • Chronic Phase: Lesions may have a well-demarcated fibrous capsule and can show complex heterogeneous collections containing degraded blood products and internal fatty debris with prominent mural nodularity.

 

✅ Differential Diagnosis

Morel-Lavallée Lesion vs. Prepatellar Bursitis:

  • Morel-Lavallée lesions are eccentrically positioned relative to the patella and extend medially or laterally to the level of the femoral epicondyles. 
  • They contain internal fat globules and persist at follow-up examinations.
  • Prepatellar bursitis, specifically chronic hemorrhagic prepatellar bursitis, may present with overlapping imaging features but does not extend beyond the prepatellar bursa.
