An osseous coalition (synostosis) is a complete bony fusion between tarsal bones, representing the end stage of the coalition spectrum.
MRI appearance
- Shows a continuous bony bridge with marrow signal extending across both bones.
- Signal follows normal marrow:
- The articulation itself disappears, replaced by uninterrupted bone.
Anatomic features
- Most often involves the middle subtalar joint.
- The joint interval may appear broadened and completely fused.
- Easily detected on both CT and MRI.
- Complete osseous coalitions are the most difficult to resect surgically.
Radiology perspective
MRI not only confirms bony continuity but also demonstrates associated remodeling. Recognizing marrow continuity is the key to confidently diagnosing a synostosis.
