🔴🇰🇷Summary of the key points regarding false positive ramp lesions in meniscal imaging:

  • 1️⃣Edema and Pericapsular Damage: A Skeletal Radiology (2018) study noted that while edema affecting the posterior capsule might indicate pericapsular soft tissue damage, the presence of edema alone is insufficient to diagnose a ramp lesion.

  • 2️⃣MRI Criteria for Ramp Lesions: According to AJR American Journal of Roentgenology (2021), no single MRI criterion is completely sensitive or specific for diagnosing meniscal ramp lesions. However, a junctional fluid-like signal is highly specific, with a positive predictive value (PPV) of 92.9%.

  • 3️⃣Common Findings in Children with Ramp Lesions: The same journal in 2021 also reported that medial meniscus tears, peripheral meniscal irregularity, junctional hyperintense signal, and meniscocapsular ligament tears were more prevalent in children with meniscal ramp lesions compared to those with intact meniscocapsular junctions.

🔴Additional Considerations:

▶️Heterogeneous High Signals on T2-Weighted Images: These signals at the meniscocapsular margin might result from tissue inhomogeneity or peripheral vessels, not necessarily indicating pathology.

▶️Fluid around the Meniscus in Post-Operative Knees: This can be a normal finding and should not automatically be interpreted as indicative of meniscocapsular tears.
