  • Diagnosing a proximal ACL tear can sometimes be difficult and may occasionally be missed.
  • ​​The normal ACL has an elliptical homogeneous low signal intensity appearance on axial images through the proximal intercondylar notch.
  • In cases of an ACL tear at the femoral attachment, axial images are particularly valuable for increased signal intensity within the proximal ACL fibers or the presence of fluid between the torn ACL and the lateral femoral condyle sidewall.
