PCL tears typically do not occur alone and are most often associated with multiligamentous knee injuries.

✅ Treatment Strategies

  • Historically, treatments have ranged from ligament reconstruction to primary repair. 
  • Recently, arthroscopic primary PCL repair has emerged as a favorable option due to its minimally invasive nature. 
  • This technique preserves native tissue, minimizes donor-site morbidity, and allows for early mobilization.

✅ Benefits of Arthroscopic Repair

  • Arthroscopic PCL repair, while not suitable for every patient, provides a viable and less traumatic alternative to reconstruction, particularly for those with proximal or distal avulsion tears. 
  • The benefits include low failure rates, improved knee stability, and generally positive subjective outcomes.

✅ Importance of Patient Selection

  • Successful outcomes from PCL repair largely depend on choosing the right patients. 
  • Ideal candidates are those with proximal or distal tears, ensuring enough remnant tissue for proper reattachment.
  • The tissue must be strong enough to endure intrasubstance suturing and tensioning back to its femoral or tibial insertion sites, which is crucial for the repair’s durability and effectiveness.
