📌 Fabellofibular Ligament

  • Appearance The fabellofibular ligament appears as a focal thickening of the distal edge of the capsular arm of the short head of the biceps femoris muscle.

  • Together with the arcuate ligament, the fabellofibular ligament functions as a static stabilizer and a restraint to external rotation, varus angulation and posterior translation of the knee. 
  • It is a potentially variable structure within the posterolateral corner of the knee.

✅ Proximal Attachment 

  • The fabellofibular ligament originates along the lateral edge of the fabella or on the posterior border of the supracondylar process of the femur, converging with the anterior fibers of the lateral gastrocnemius origin.
  • When the fabella is present, it anchors the fabellofibular ligament, which stretches from the fabella to the styloid process of the fibular head posterior to the arcuate ligament and lateral to the popliteofibular ligament. 
  • If the fabella is absent, the fabellofibular ligament attaches to the lateral femoral condyle.

 

✅Distal Insertion 

  • The distal insertion is on the fibular styloid process, posterior to the lateral bundle of the arcuate ligament and the biceps femoris tendon, and lateral to the popliteofibular ligament.

 

✅Inverse Relation to Arcuate Ligament 

  • The presence, size, and contributions of the arcuate and fabellofibular ligaments are inversely related. The fabellofibular ligament (when present) functions as the lateral limb of the arcuate ligament.

 

✅Vascular Relation 

  • The lateral inferior geniculate vessels course between the arcuate and fabellofibular ligaments.
