📌 MPFL Complex Overview
1️⃣ Transverse MPFL Component
- Ribbon-like, horizontal orientation
- Spans from medial patella (proximal two-thirds) to medial femur
- Attaches near physeal scar, between femoral epicondyle and adductor tubercle
2️⃣ Oblique Decussation MPFL Component
- Attaches to proximal superficial medial collateral ligament fibers
- Recently described
Clinical Significance
- Abnormal MPFL appearance on MRI in superficial medial collateral ligament injuries
3️⃣ MQTFL (Medial Quadriceps Tendon Femoral Ligament)
- MPFL extension attaching to quadriceps tendon
4️⃣ MPML (Medial Patellomeniscal Ligament)
- Extends from inferomedial patella to medial meniscus
5️⃣ MPTL (Medial Patellotibial Ligament)
- Extends from inferomedial patella to anteromedial tibia
- Distal patellar insertion on medial side
- Close to patellar tendon and articular cartilage
✅ Attachments
- MPML meniscal insertion: anterior horn or body transition
- MPTL tibial insertion: Medial tibial tubercle, ~5 mm distal to joint line
