📌 Case study: O'Donoghue's Triad

  • The O'Donoghue unhappy triad refers to the combined injury of the anterior cruciate ligament (ACL), medial collateral ligament (MCL), and meniscus
  • Originally described with medial meniscal injury, but lateral meniscal tears more common when ACL and MCL injuries coexist
  • Occurs from contact or non-contact injuries

✅ MCL Injury Findings

  • Thickening of MCL
  • Increased internal signal intensity
  • Surrounding soft tissue edema
  • Case shown is Grade 2 MCL injury

✅ Meniscal Tear Findings

  • Linear high T2 signal through meniscus from superior to inferior articular surface
  • Represents complete longitudinal vertical tear in red-red zone
  • Classified as ramp lesion subtype 4A

✅ ACL Injury Findings

  • Irregular, cloud-like area of high signal in ACL substance
  • Loss of normal striated fiber appearance
  • Indicates complete tear
